Required Participation Forms for Sports
In the below url links, you will find several pieces of information that you and your student will need to read and fill out prior to them participating in any athletic practice. You need to make sure the school has a copy of your students physical, a signed NSAA consent to participate, an emergency contact information formed fill out and signed, and the parents guide to concussion sheet. Again, all of this will need to be read, filled out and signed prior to practice.
In January of 2012 the Nebraska Legislature signed into affect LB620. This bill went into affect on July 1, 2012. As part of this bill each Nebraska school is required to give each parent a copy of the "parents guide to concussions", please read through this information with your students. Anselmo-Merna Public Schools will also have a coach trained in concussion testing for each sport. Your student will be pre-tested in the case you or the coach suspect a concussion during a practice or game. They will be tested again to determine if there is in fact a possible concussion. If your student is suspected of having a concussion the following steps will take place: 1) A coach will contact you giving you details of the signs they saw during the evaluation along with the time of the suspected concussion. 2) The coach will document this and it will be kept on file at the school. 3) Before returning to practice they will need to be cleared by a medical professional with a signed release. 4) They will also need to bring a signed release from you the parent, this will be sent home with the student in the event of a suspected concussion.
